How old were Fred and George when they left Hogwarts, and what year were they in?
I'm pretty sure they were in third year the year Harry went to Hogwarts, so they are 2 years older, meaning they were in 7th year in OOTP.

Fantasy book with an incompetent young magician
Maybe add the name of the saga, The Riftwar Saga. That was my answer as well, but the school part made me not put it as a real answer

Fantasy book with an incompetent young magician
Could it be The Riftwar Saga? The first book depicts a boy running from the weather, and meeting a wizard. As the boy is supposed to be taken as an apprentice in the castle he lives, the wizard decide to take him when no one wants to. But Pug (the boy's name) can't seem to use magic as the wizard can; he seems to have difficulties to use spell at will, but can be really powerfull in case of danger. There is no school though.

How is the team name order determined for House Quidditch games at Hogwarts?
I don't get the second half of your question. What does the fact that Slytherin and Gryffindor being both Hogwarts team come in the order of the game?

Live action TV show where High school Kids go into the virtual world and have to clear levels
@TinyDoowy Code Lyoko, in cartoon, went out of France, it was even available on US Netflix 'til 2015. According to Wikipedia, Code Lyoko Evolution was diffused in France, Belgium, Hungary and Serbia. There is a date of diffusion for the fifth episode for the US, but the reference seems to be outdated.

Why did the Ministry of Magic allow Dumbledore's nepotism/ cronyism?
Well, Snape and Slughorn were both really competent potions maker (and competent teacher, when they feel it that way). Lupin was there because finding a DADA teacher is really difficult when they know the post is cursed. Trelawney made a really important prophecy and needs to be protected. We don't really have informations if the MoM tried to intervene before OOP, but I guess that Umbridge was quite an intervention, no?

Fantasy book about an (orphan?) girl named Opal, involves gems or stones
I read the title and the description, and I was absolutely sure it was the Prophecy of the stones. Are you sure you didn't mix it up with another book?

How many lies did Ned Stark tell?
@TheLethalCarrot concerning Jon true lineage, he often says "You're of my blood", which is true, and he did have an affair with a woman, more supposed to be with Ashara (Wylla's mistress) who commited suicide after the death of their baby. He would however have lied about the identity of his lover to Robert. The article about this is sadly in french, I don't know where to find its equivalent in english : lagardedenuit.com/de-parente-de-jon-snow EDIT : these informations only stand for the books

Is there an in-universe scientific explanation for the Engineers having white skin?
A movie analyst I am following thinks that the Engineers were made to look like the marble statues from antic Greece and Rome, so we could supposed these statues were made to represent the Engineers and prove they went on Earth before.

Short story, older people who can't separate mind from body
From what is told, I can only think of His Dark Materials by Phillip Pullman. The first book is set in an alternative world Oxford, in which people have their soul under the form of animals, called daemons. The daemons of the youngs still change of form until they reach the age of what they know what they're going to be, and the form stays until they die. The story of the first book revolves around the fact of adult wanting to separate children from their daemons to get the dust.
